| 中文摘要: | 区域医疗资源配置不均衡和公立医院改革的问题是当面中国医疗卫生体制改革的突出矛盾。为破解当前医疗资源分配不均的问题,有必要采取医联体的组织管理模式,探索大型公立医院的发略发展的创新模式,在破解公立医院发展改革问题的同时,促进区域医疗资源的合理分配和使用,实现合理分流患者和优化医疗资源配置。 本研究的主要针对对象是以TCH医院为代表的大型公立医院。通过专家访谈法并结合PEST、SWOT和波特五力模型等战略分析工具,对TCH医院进行从宏观的产业视角到微观的个体视角进行现状分析,指出TCH医院当前发展的核心问题在于:因资源空间限制和虹吸效应,TCH医院的大多医疗资源被常见病、慢性病和非手术患者占据,影响了医院吸纳危重症、手术患者等复杂病源和新病源的能力,从而造成了TCH医院技术实力提升障碍,进一步影响了该院的经营效益。 针对该问题,通过文献研究并针对性的选择实地考察,制定了基于医联体的战略发展模式作为解决方案。以TCH医院为中心,联合周边基层医院组成医联体,形成慢性病和常见病到基层医院,疑难症危重症患者到三级医院的分级诊疗目标。在提升TCH医院病源质量、破解了发展瓶颈的同时,也带动了区域整体医疗水平的提升。 为确保方案顺利实施,设计了医疗、医技和后勤物流为三条纽带,并建立了理事会型治理结构和基于云数据的信息平台作为支撑。本研究还创新的提出了将基层医院作为三级医院的客户,利用三级医院的优势资源破解基层医院发展难题这一新型医联体合作模式。并且,使用顾客价值理论构建了医联体“全流程的健康管理”服务,提高患者满意度和忠诚度,最终形成大型公立医院、社区医院、患者群体和政府四方满意的效果。 | |
| 英文摘要: | The imbalance of medical resource allocation and reform in public hospital are the prominent contradiction of Chinese medical and health system reform. It is necessary to take hospital alliance as the solution, in order to solve the problem of development and reform of public hospitals and the problem of current medical resource distribution. It would finally achieve a community-level clinics system and optimize the allocation of medical resources. The main target of this research is the large public hospital, which is represented by TCH hospital. Through expert interview method combine with three types of strategic analysis tools, such as PEST, SWOT and Potter's five forces model, the research have made an analysis for the current situation of TCH hospital from both macro perspective and micro perspective. It pointed out that the core problems in development of TCH hospital is that most of the medical resources in TCH hospital are occupied by common patients, chronic patients and non-operation patients, due to the resource space limitation and siphon effect. Those result in the fact that the ability and spacious to accept critically ill patients and complex operation patients, further more lead to the suspending in improvement of the technical strength of the TCH hospital, and finally affect the operation benefit of the hospital. Through literature research and field investigation, the strategic development model based on the hospital alliance system is formulated as a solution, in order to achieve the goal that patients with chronic diseases and common diseases receive medical services from primary hospitals, while patients with Critical illness receive medical services from TCH hospital. It would not only breakthrough the development bottleneck of TCH hospital, but also promote the regional medical standard. In order to ensure the smooth implementation of the plan, the research designed clinic, medical laboratory, logistics system, and council governance structure and information system. The cooperation model of the development of the primary hospital with the advantage of the three grade hospital is put forward. It issued that using the advantage resource from TCH hospital to solve the problem of the development of community hospital and using the customer value theory to establish the whole process health management system as another innovation point. | |
