中文摘要：	在中国进入老龄化社会的背景下，我国的公共医疗卫生体系面临着越来越大的压力和挑战。如何实现老年患者持续无缝隙疾病护理、康复护理和家庭保健护理，合理调配公共医疗卫生机构和优质护理的保障资源，有效发挥大型（综合）医院、社区医院、家庭对老年患者的护理职责，是摆在我们面前亟待解决的课题。本论文尝试借助精细化管理、顾客服务满意度与服务质量、顾客忠诚及服务质量等管理思想与现代医院、社区、家庭管理相结合，在此基础上力图通过现代化通信网络、医院（社区）信息化管理系统、智能医疗设备等科技手段，为大型（综合）医院、社区、家庭构建全方位的联系网络平台和信息共享平台，通过横向、纵向网格化护理信息和护理服务的延伸，在医院、社区、家庭之间建立密切的联系和有效的沟通，从而实现“医院—社区—家庭”老年全程精细化护理服务的愿景目标，为公共医疗卫生机构老年护理精细化管理、老年护理教学培训、老年护理科研创新、老年护理学科人才队伍建设构建一个全新的护理模式。本研究目的一是充分利用大型（综合）医院优质的护理资源，最大限度发挥医院对社区医疗机构老年护理专业技术的支持；二是发挥社区医院的地域优势，实际参与或指导家庭老年护理工作，减轻对综合医疗机构的长期依赖，从而达到解决日益紧迫的老年社会带来的压力；三是积极倡导老年人日常家庭保健，提高老年人生活质量。本研究以301医院的精细化护理、信息化建设、智能化保障支持系统等成熟经验为基础，前瞻性探索未来社会老年护理服务模式。希望通过理论联系实际，为创新老年护理服务模式提供可借鉴的管理方案。随着经济社会的快速发展，老年医疗护理工作不仅代表着一个国家的医疗卫生水平，更体现社会文明的发展与进步。
英文摘要：	As China entering an aging society, our public health system is facing more and more pressure and challenges. There are a few urgent issues that require our attention and our efforts: 1. How to accomplish continuous and seamless disease-nursing, recovery-nursing and health-care-nursing for the elder. 2. How to efficiently operate public health organizations and arrange high quality nursing resources. 3. How to effectively utilize the nursing resources for the elder patients provided by large (multi-functional) hospitals, community clinics and families. In this paper we combine delicacy management, customer satisfaction and service quality management and customer loyalty and service quality management with modern hospital-community-family management to improve our public health system. Moreover, we utilize modern communication network, electronic management system and intelligent medical equipment to construct comprehensive communication networks and information-sharing platforms for large (multi-functional) hospitals, communities and families. Through a three-dimensional network of nursing information and nursing services, we try to build a close relationship between hospitals, communities and families and realize effective communications between them. As a result, we try to accomplish our goal of providing elaborative nursing services all the way from hospital to community then to family. Finally, we provide a new model for delicacy management, training, research and professional team-building in elder-patient-nursing. This research addresses three main topics. Firstly, through high quality nursing resources provided by large (multi-functional) hospitals, we try to maximize the technical supports on elder-patient-nursing provided by large hospitals to community clinics. Secondly, we suggest making use of the regional advantage provided by community clinics, participate in or provide instructions on elder-patient-nursing to families. Thus, we can reduce the on-going pressure on large hospitals and overcome difficulties caused by the aging society. Thirdly, we encourage daily family health care to provide a better life for the elder. This research is based on our experience in delicacy nursing, electronic information system and intelligent security and supporting system at 301 Hospital. We creatively explore the future model of elder-patient-nursing. By combining the theory with applications, we wish to provide a practical guide for innovative elder-patient nursing and services. With the rapid development of our economy, elder-patient-nursing not only represents the development of public health in China but also indicate the progress and improvement of our society.
