中文摘要:近年来,精益管理研究从制造业逐渐向其他管理领域延伸,尤其在医疗服务行业的发展正方兴未艾,逐渐形成以精益医疗为特征的管理模式。然而,在某些民营的专科医疗服务领域,如民营的眼科医院,有关精益管理的研究仍相对较少。民营眼科医院的门诊诊疗流程具有其特殊性,其检查的顺序、检查的项目、预约手术的流程等都与其他疾病科室不同,甚至与公立综合性医院的眼科诊疗流程也有差异,而患者对民营眼科医院的服务水平又有更高的期待。因此,我们在民营眼科医院门诊诊疗流程中引入精益管理研究,以消减其门诊诊疗流程中的非增值活动,实现精益优化,提升效率,改善患者满意度,具有一定实践意义和研究价值。 本研究以A民营眼科医院的白内障门诊诊疗流程为例,进行精益管理研究。以精益管理理论、流程管理理论作为主要理论依据,运用文献研究法、实地调研法、案例分析法展开研究。采用多种精益管理、流程管理优化工具和方法,如美国机械工程师学会(ASME,American Society of Mechanical Engineers)标准流程分析法、价值流图分析法、精益小组讨论等,对A民营眼科医院的白内障诊疗流程进行拆解和分析,解决流程中的非增值活动的浪费问题,并采用流程重组、可视化工具、数字化工具等设计出门诊流程的精益管理优化方案,为民营眼科医院的门诊流程优化提供一种解决问题的案例和可行方法。本研究还进一步提出A民营眼科医院的精益管理优化方案实施的保障措施,阐述了A眼科医院白内障门诊流程精益管理优化方案保障实施的关键因素、完善精益管理团队建设和精益管理优化方案的效果评估等内容,最后提出研究结论及不足。 本研究针对国内民营眼科医院的门诊诊疗流程进行精益管理研究,将流程管理理论和精益管理理论结合,应用多种理论工具来分析问题,辅助精益管理研究,在研究方法上有一定的新颖性,并提出了一套可以实践的门诊流程精益优化方案,对民营眼科医院进行精益管理研究有一定的参考和实践意义。本文含图12幅,表5个,引用参考文献48篇。 关键词:眼科医院;门诊流程;精益管理;流程管理 
英文摘要:In recent years, lean management research has gradually extended from manufacturing to other fields, especially in the development of the medical service industry, which has formed a management model characterized by medical lean management. However, in some private specialized hospitals, such as an eye hospital, there are still relatively few researches on lean management. The outpatient process of a private eye hospital always has its own particularity. The examination process, items, and procedures for making an appointment for surgery are different from other departments, and even different from the ophthalmic department in public hospitals. There are also higher expectations for the service level of private eye hospitals. Therefore, we introduce lean management research into private eye hospitals to eliminate non-value- activities in the outpatient process, achieve lean optimization, improve efficiency, and patient satisfaction, which has certain practical and research value. This study takes the cataract outpatient process of A private eye hospital as an example to conduct lean management research. Taking lean management theory and process management theory as the main theoretical basis, the research is carried out by using academic papers research, on-site research method and case analysis method. Using a variety of lean management and process management optimization tools, such as American Society of Mechanical Engineers (ASME) standard process analysis, value stream mapping analysis, on-site group discussion, visualization tools, etc.,the outpatients process of A private eye hospital was disassembled and analyzed to eliminate the waste of non-value activities in the process, so as to design a lean management optimization plan for the process, and provide a problem-solving case and feasible method for the optimization of outpatient process in private eye hospitals. This study further proposes the implementation and safeguard measures of the lean management optimization plan of A private eye hospital, expounds the key factors of the implementation of the lean management optimization plan for the cataract outpatient process of A eye hospital, and improves the lean management team building and lean management optimization plan. and so on. Finally, the conclusions and shortcomings of the study are presented. In this study, a lean management study was conducted on the outpatient process of domestic private ophthalmology hospitals. It combined the process management theory with the lean management theory, applied a variety of theoretical tools to analyze problems, and assisted the lean management research. The research method has a certain novelty. And put forward a set of practical outpatient process lean optimization plan, which has certain reference and practical significance for the lean management research of private eye hospitals.
