中文摘要:党和国家颁布了一系列关于提高健康管理、改善健康服务、促进全民健康的政策文件,全民健康是我国全面实现小康社会的重要基础和主要标志之一。提升公民健康信息素养(Health Information Literacy,HIL),塑造有利于获取与利用高效健康信息的行为十分必要。为此,本研究分析了不同健康信息素养水平用户的健康信息搜寻行为特征,并构建了健康信息搜寻行为路径的理论模型。 主要的研究工作包括:第一,针对疾病类健康信息的搜寻,回顾已有文献,通过半结构化深度访谈探究健康信息素养的维度;第二,基于得出的健康信息素养维度,设计问卷,通过问卷调查法调查用户的健康信息素养情况,并采用独立样本T检验、单因素方差分析及卡方检验等方法探究影响用户健康信息素养的因素;第三,采用跟踪访谈法,针对问卷调查的结果,通过开放编码,深入分析不同健康信息素养水平用户的健康信息搜寻行为特征,构建健康信息搜寻行为路径的理论模型。 本研究得出以下研究结论: 一、用户健康信息素养共包含七个维度,分别是:信息获取能力、信息评估能力、跨源搜寻能力、习得知识、寻求专业帮助、搜寻终止和元认知维度。 二、不同健康信息素养水平用户具有不同的行为特征。拥有高健康信息素养的用户更倾向于选择网络搜寻健康信息,而健康信息素养水平较低的用户更倾向于通过去医院就医搜寻健康信息。不同健康信息素养水平的用户对健康信息源的选择不同,选择原因也不同。此外,这三类用户(高、中、低健康信息素养的用户)对健康信息质量的评估有不同的依据,其跨源意识、跨源动机也不相同,并且对待网络信息的态度也不同。 三、不同健康信息素养水平用户的健康信息搜寻行为路径并不相同。他们拥有不同的初始搜寻动机,由于不同的原因选择不同的信息源搜寻健康信息,通过不同的评估策略对信息质量进行评估后形成不同的决策,又依据不同的习得知识能力、获取专业帮助意识形成不同的跨源动机,出于不同的信息需求选择不同的信息源跨源搜寻健康信息,又因不同的信息质量评估策略形成不同的决策方案。 四、不同健康信息素养水平用户对网络健康信息的定位不同。高健康信息素养的用户通常认为网络健康信息对自己的健康决策起辅助的作用,中等健康信息素养水平的用户则会将网络健康信息纳入自己的健康决策中,而低健康信息素养水平的用户则对网络健康信息有强烈的防备心。 五、影响健康信息素养水平的因素包括年龄、学历、家庭年收入等人口统计学变量因素及用户自身的健康状况、对身体的关注程度等。 六、无论健康信息素养水平的高低,用户普遍缺乏对自身认知的自省,即元认知活动。相对来说,博士学历用户的元认知能力显著强于其他学历的用户。 本研究从用户真实的健康信息搜寻经历入手,分析符合用户需求的健康信息素养维度,构建健康信息素养理论模型。此外,对比揭示了高、中、低健康信息素养水平用户搜寻行为特征的不同和健康信息的搜寻路径,为日后更有针对性地提升用户健康信息素养提供理论基础,也为更加符合用户需求的健康信息系统的设计和开发提供了实证依据。基于以上结果,本研究构建了不同健康信息素养水平用户的信息搜寻行为路径理论模型,丰富了健康信息素养和健康信息行为的研究内容。  
英文摘要:The Party and the country have promulgated a series of policy documents on improving health management and services in order to promoting the health of the people. National health is one of the important foundations and main signs of realizing a well-off society in an all-round way. It is necessary to improve the citizens' health information literacy and to shape the behaviors which are conducive to the acquisition and utilization of high-efficiency health information. Therefore, this study constructs the model of behavior path of health information seeking . The main research work includes: firstly, review the existing literature and explore the dimensions of health information literacy through semi-structured in-depth interviews; secondly, based on the health information literacy dimensions, we design a questionnaire to survey the level of users’ health information literacy, and use Independent-samples T test, One-way ANOVA analysis and Chi-square test to analyze the data to reveal the influence factors of health information literacy. The third, follow-up interview is performed to collect the data about users’ health information seeking experiences. Open coding is performed to analysis the follow-up interviews’ data to analyze the behavior characteristics of different health information literacy users, and to establish the theoretical model of behavior path of health information seeking. The following conclusions are drawn in this study: 1.There are seven dimensions of health information literacy, which are: health information seeking ability, health information evaluation ability, cross-source seeking ability, acquisition knowledge, seeking professional help, seeking termination and metacognition. 2. Users with different health information literacy have different behavioral characteristics. Users with high health information literacy tend to search for health information on the Internet, while those with lower health information literacy tend to go to the hospital to seek health information. Different users of different health information literacy level choose different sources of health information, and the reasons for the selection are different too. In addition, the three types of users (with high, middle, and low health information literacy) have different basis for the evaluation of health information quality, and have different cross-source motivation awareness and motivation Besides, their attitudes towards online information are different too. 3.The behavior path of health information search is different among users with different health information literacy levels. They have different initial search motivation. For different reasons, they choose different information sources to seek health information, and then evaluate the information quality through different evaluation strategies and get different decisions. They also form different cross-source motives based on different knowledge acquisition ability and professional help awareness. Different sources are chosen to cross-source seeking because of different information needs. And different decision-making schemes are made due to different information quality evaluation strategies. 4.Users with different health information literacy levels treat online health information in different ways. Users with high health information literacy usually make clear that network health information plays an auxiliary role in their health decision-making. Users with medium health information literacy level will incorporate online health information into their health decision-making, while users with low health information literacy strongly suspected. 5.The factors which affect the level of health information literacy include age, education background, family annual income, residence, the health status of users, and the degree of the concern to the health. 6.No matter the level of health information literacy, users generally lack self-examination of their own cognition, which is metacognitive activities. Compared with low education users, high education users’ metacognition is stronger. This study starts with users’ health information seeking experience, analyzes the dimensions of health information literacy that meet the needs of users, and constructs a theoretical model of health information literacy. In addition, the study reveals the different behavior characteristics of users with high, medium and low health information literacy levels and the search path of health information, which provides theoretical basis for more targeted improvement of user health information literacy in the future, and provides empirical basis for the design and development of health information system which better meets the needs of users. Based on the above results, this study constructs the theoretical model of behavior path of health information seeking. It enriches the research content of health information literacy and health information behavior.
