中文摘要:公立医院作为国民生活中的重要组成部分,是国家提升人民生活幸福指数的改革关键点。在医改的大背景下,想要加强自身建设,增加医疗收入,提升综合竞争力,同时更好的满足人民的医疗需求,公立医院就需要不断的购置及更新与自身诊疗能力相适应的大型医疗设备。 公立医院大型医疗设备的购置,在医院的购置体系中,往往地位重要,金额巨大。然而,以药养医的传统和非营利性医疗机构的特性影响,公立医院的大型医疗设备购置的成本管理一直相对粗放。采用招投标模式进行设备评估选购,在设备成本评估中,管理者往往只注重初始购置成本,对后续的运行成本及保养维修费用的关注甚少,而大型医疗设备的运行成本和保养维修费用实际金额较大,这种对运行成本和保养维修费用估测的忽略,无法科学准确的估算大型医疗设备的实际成本,往往使医院被金额较低的购买价格所迷惑。 本文基于公立医院大型医疗设备购入中存在的“冰山效应”,即对运行成本和保养维修费用的忽略,针对大型医疗设备购置中存在的具体问题,提出引入全生命周期成本法的可行性及必要性,对大型医疗设备全生命周期成本进行详细分析,将全生命周期成本法应用于具体大型医疗设备购置,实现新购置设备全生命周期成本最小化。 本文还提出,为保证全生命周期成本法在医院设备购置中成功运用,在公立医院的设备购置活动整个流程中,需要进行改进和标准化的措施,包括完善数据支撑、构建供应商评价体系、梳理招标活动和改进评标指标,以服务于全生命周期成本法科学有效的应用于公立医院大型医疗设备购置。 目前国内对于全生命周期成本法应用于公立医院大型医疗设备购置领域的研究相对较少,本文希望能为公立医院大型医疗设备购置经济性的发展,起到一定的推动借鉴作用。 
英文摘要:As an important part of national life, public hospitals have always been the key point in the reform of improving people's life happiness index. In order to strengthen self-construction, increase medical income, enhance comprehensive competitiveness, and better meet the medical needs of the people, public hospitals need continuous investment to purchase and update large medical equipment suitable for their own diagnostic and therapeutic capabilities. The investment of large medical equipment in public hospitals often plays an important role in the investment system of hospitals and needs a huge amount of money. However, the cost management of large medical equipment purchases in public hospitals has been relatively rough due to the influence of supporting medicine with drugs tradition medical system and the characteristics of non-profit organizations. Applying bidding mode to evaluate and purchase equipment, in equipment cost evaluation, managers often only pay attention to the initial investment purchase cost, pay little attention to the follow-up operation cost, and the actual operation cost of large medical equipment is larger. This neglect of operation cost estimation cannot scientifically and accurately estimate the actual cost of large medical equipment. Managers always be confused by the lower initial investment purchase cost. The paper focuses on the iceberg effect existing in the purchase of large medical equipment in public hospitals, as the neglect of operating costs, put forward the feasibility and necessity of introducing life cycle cost management in the specific problems existing in the investment of medical equipment, analyze the life cycle cost of medical equipment in detail, and the life cycle cost method is applied to the purchase of specific large medical equipment to minimize the life cycle cost of newly purchased equipment . The paper also comes up with the idea that In order to ensure the successful application of the whole life cycle cost method in the purchase of hospital equipment, the measures which need to improve and standardize in the whole process of the purchase of public hospital equipment, including improving the data support, building the supplier evaluation system, sorting out the bidding activities and improving the evaluation indexes to serve the scientific and effective application of the life cycle costing in hospital equipment investment. At present, there are relatively few studies on the application of life cycle cost method in the field of large medical equipment investment in public hospitals in China. This paper hopes to provide some reference and impetus for economic development of large medical equipment investment in public hospitals.
