中文摘要: | 随着中国与世界发达国家经济接轨的程度越来越高,国内消费者对于产品质量和服务过程的要求逐步升级。面对这些客观因素,国内制造型企业、服务业以及医疗行业的众多经营者和管理者,开始思考在新的市场环境下,如何由以前的单纯的追求产量的粗放管理向精益管理转变。虽然精益思想的理念始于制造型企业,但随着各个行业的专家学者参与到精益的研究与实践,精益已应用到各个行业中,包括金融、设计、服务业、医疗等。中国作为人口大国,面临人口老龄化和医疗资源不足的问题,普通患者就医体验相对较差,患者在医院消耗大量的时间在排队办理各类事务上,而真正花费在面诊和治疗的时间相对较少,并因此引发了医患矛盾等社会问题。这些问题引起了国内诸多医疗行业管理人员的重视,很多医院开始学习和研究如何打造一家精益医院。针对上述问题,本文以医院较为基层但与患者关系密切的护理部为切入点,使用现场观察法和非结构访谈法,绘制出护士工作流程图、采集护士工作节点、计算护士工作量。通过计算护士工作负荷效率,实现了护士工作的量化评估。通过对各科室护士负荷效率的计算以及各科室护士负荷效率的横向比较,有利于医院护理部人力资源的优化配置,减少医院非必需的人力资源投入。在护理部工作量化的同时,通过对医院护士工作流程的精益评估,给出精益改善建议方案,并在方案实施后再次评估其实施效果。这一精益医疗实践过程,对医院其他部门和科室均有借鉴意义,并且对医院整体的运营效率的提升起到了促进作用。 | |
英文摘要: | Along with CHINA gigantic grows, chasing the steps with developed country, Chinese customers pay more and more attention on the quality of product and services. In order to survive in the new market environment, management of internal enterprise, service industry and medical industry began to change the operation from extensive management to lean management. Although the lean concept developed from manufacturing industry, with more and more experts come into the research and practice on the wildly professions, lean management have been extended to all the industries, including manufacturing, designing, service and medical industry. China is a top-population country with aging population and insufficient resources, the patient experience in hospital is un-satisfied due to the long time on wait and less time on treat. The contradiction between patient and doctor is getting harder. The hospital management noticed the issue and started to find a way to build a lean hospital. Under this circumstance, this thesis implemented the lean management into the nursing department whom patient spent most time with during the diagnosis and treat. Using the field observation method and unconstructive interview, the nursing job overall efficiency is calculated by drawing the work flow chart, recording the work note and the time costed. The nursing overall work efficiency quantized the nurse’s workload, which can be used for the nurse department human resource re-allocation optimization and hospital human cost reduction. After the human resource re-allocation, the lean improvement practice is designed and started in the nursing department. Most of wastes are found and adjusted by the certain experiment unit, it is successful, can be expanded to other nursing department and even the whole hospital. | |
