中文摘要:从1985年医疗改革正式开始到现在,中国的医疗改革已经历时30年,经 历了6次大的改革。回顾中国医改三十年,我国的基本医疗保障制度得到了全面的推广,各地区逐步建立起较为完善的基层医疗卫生服务体系,城市与农村所提供的基本公共卫生服务水平差距正在不断缩小。与此同时,社会办医也成为了新医改的重要工作内容。而一方面,我们的医疗改革也面临着一些还未得到有效解决的问题,城市卫生资源的配置结构呈头重脚轻的“倒三角”分布,基层医疗卫生机构的资源不足、人才素质不高、管理方式粗旷等问题大大影响到了其医疗服务功能的发挥。建立基层首诊、双向转诊、急慢分治、上下联动分级诊疗制度则是医改成败的关键。社区医院作为基层医疗机构的重要组成部分如何最好居民健康“守门人”,建立一支高素质的基层医务团队至关重要。 本研究以唐山市某民营社区卫生服务机构为例,通过问卷调查、现场访谈、数据统计整理、现场观察等多种调研手段分析得出本社区卫生服务机构在人员管理上存在的人员结构、培训发展、薪酬体系、绩效管理方面的问题。针对以上存在的问题及该民营社区卫生服务机构的特点,以人力资源及管理及社区卫生服务的相关理论为理论依据,结合国家政策及该社区卫生服务机构的现状在组织架构、人才吸引、培训发展、绩效管理、薪酬体系建设等方面提出优化方案。结合该社区卫生服务机构的发展规划以及其投资人的海外背景,建议在未来可针对以下几点进行更加深入的研究和尝试:探讨激发外聘专家传帮带作用发挥的有效方法;如何通过工作嵌入去实现员工留任及绩效改进;利用其投资人的海外及卫生系统内的资源进行多元化培训资源的对接以提高医护团队学习及获得资源的广度和深度、进行信息化建设以便为患者提供更加多元和高质量的诊疗资源和形式。该研究落脚于民营社区卫生服务机构,对该社区卫生服务机构的人员管理提升具有现实的指导作用,同时也对同类基层医疗的人力资源管理有着一定程度的借鉴意义。 
英文摘要:From the beginning of medical reform in 1985 to the present, China's medical reform has lasted for 30 years, has experienced six major reforms. Recalling that in the past 30 years of Chinese medical reform, China's basic medical security system has basically been fully covered, and the basic medical and health services system has been constantly improved. The process of urban-rural equalization of basic public health services is gradually advancing. At the same time, social medical care has become an important part of the new medical reform. On the one hand, our health care reform also faces some problems that have not yet been effectively solved. Configurations's structure of urban health resources is a inverted triangle distribution of top-heavy and top-heavy。The shortage of resources, the low quality of talents and the rough management style of primary medical and health institutions have greatly affected the development of their medical service functions. The key to the success or failure of medical reform is to set up the system of first diagnosis, two-way referral, acute and chronic treatment, and the system of combined diagnosis and treatment. As an important part of the primary medical institutions, it is very important to establish a high-quality primary medical team in community hospitals. Taking a private community health service organization in Tangshan city as an example, this study analyzed the problems of personnel structure, training and development, salary system and performance management in the community health service organization by means of questionnaire survey, field interview, data statistics and on-the-spot observation. In view of the above problems and the characteristics of the private community health service organization, based on the relevant theories of human resources and management and community health service, combined with the national policy and the current situation of the community health service organization, this paper puts forward the optimization plan in the aspects of organization structure, talent attraction, training development, performance management and salary system construction. Based on the development plan of the community health service and the overseas background of its investors, it is suggested that further studies and attempts can be made in the future:To explore the effective methods to stimulate the role of external experts to play; How to achieve employee retention and performance improvement through job embeddedness;To make use of the overseas resources of its investors and the resources of the health system to carry out diversified training resources, so as to improve the breadth and depth of medical team learning and obtaining resources, and to carry out information construction so as to provide patients with more diversified and high quality medical treatment resources and forms. This study is based on the private community health service agencies, which has a practical guiding role in improving the staff management of the community health service agencies.
